Services: contact us and newsletter subscription Article 13 of EU regulation n. 2016/679 (“GDPR”)
A-Life Milano S.r.l. (hereinafter also referred to as “A-Life”, the “Controller”, or the “Company”), in accordance with the provisions regulating the protection of personal data, namely with article 13 of EU Regulation No. 2016/679 (“GDPR”), hereby provides you with the following information regarding the processing of your personal data (hereinafter, “Data”) in connection with the management of your information request or contact submitted to the Company through the dedicated contact form available on the website https://a-life.care/en/.
1. DATA CONTROLLER
The Data Controller is A-Life Milano S.r.l., with its registered office at Via della Guastalla 2, Milan, Italy, VAT/Tax Code 13625690964. The Controller may be contacted at the following e-mail address: privacy@a-life.care.
2. PERSONAL DATA PROCESSED
The Controller collects and processes the following common personal data: first name, last name, date of birth, e-mail address, telephone number, city and country of residence.
3. DATA SOURCE
The Data are collected directly from you, as the data subject, when you submit your request for information/ newsletter subscription through the dedicated forms.
4. PURPOSE OF THE PERSONAL DATA PROCESSING
Your Data are processed by the Company for the following purposes:
- Purposes related to handling and responding to your request
The legal basis for the processing of common Data is the performance of pre-contractual measures taken at the request of the data subject, pursuant to Article 6(1)(b) of the GDPR.
- General marketing purposes
Sending, via digital means (such as WhatsApp, SMS, and e-mail, including automated tools) and/or traditional means (such as operator-assisted calls and postal mail), of promotional and commercial communications concerning A-Life’s services and activities, as well as information about events (both online and in person) organized by A-Life. The legal basis for this processing is your consent, pursuant to Article 6(1)(a) of the GDPR. Your consent is optional and may be withdrawn at any time.
5. NATURE OF THE PROVISION OF THE PERSONAL DATA AND CONSENT
For the purpose referred to under letter a) above:
- Providing the Data marked with an asterisk (*) is mandatory in order to enable the Controller to process your request submitted through the contact form. Failure to provide such Data will prevent the Controller from responding to your message.
- Providing the Data not marked with an asterisk (*) is optional; therefore, failure to provide them will not affect the Controller’s ability to handle and respond to your request.
For the marketing purpose referred to under letter b) above, the processing of your Data will only take place if you have expressly opted in by ticking the corresponding checkbox. You may withdraw your consent at any time, without providing any reason.
6. DATA RETENTION PERIOD
Data processed for the purpose under letter a) will be retained:
- for 24 months from the submission of your initial request, if no further interaction with the Company occurs; or
- for the duration of the contractual relationship, should you decide to use A-Life’s healthcare services (e.g., by making a booking), thus becoming an A-Life patient. In that case, you will be provided with a separate and specific privacy notice regarding the processing of your data as an A-Life patient.
Data processed for the purpose under letter b) will be retained for a maximum of 24 months from the date of your subscription, unless you withdraw your consent earlier.
7. DATA RECIPIENTS
- The Data may be disclosed to entities acting as independent Data Controllers, including public authorities or other public or private entities legally entitled to request the Data.
- The Data may also be processed on behalf of the Controller by entities designated as Data Processors, pursuant to Article 28 of the GDPR, including, in particular, companies providing website maintenance services.
Authorized Personnel
The Data may be processed by employees and collaborators of the Company specifically authorized by the Controller, who have received appropriate operational instructions and are bound by professional confidentiality obligations.
8. TRANSFER OF PERSONAL DATA OUTSIDE THE EUROPEAN ECONOMIC AREA (EEA)
Your Data will not be transferred outside the European Economic Area under any circumstances.
9. DATA SUBJECT RIGHTS
By contacting the Controller at privacy@a-life.care, you may, at any time, exercise the following rights:
- withdraw your consent;
- request access to your Data (Article 15 GDPR);
- request rectification or completion of your Data (Article 16 GDPR);
- request erasure of your Data (Article 17 GDPR);
- request restriction of processing (Article 18 GDPR);
- receive your Data in a structured, commonly used, and machine-readable format, and, where technically feasible, have them transmitted to another controller without hindrance (“data portability right”, Article 20 GDPR).
You also have the right to object, at any time and free of charge, on grounds relating to your particular situation, to processing based on the Controller’s legitimate interests.
Finally, you have the right to lodge a complaint with the competent supervisory authority pursuant to Article 77 of the GDPR, if you believe that the processing of your personal data violates applicable data protection law.
